Statistical methods that serve to represent, based upon population parameters of mean and variance, for one, tow or various samples taken from study populations are discussed. Ways to reduce the amount of experimental error in order to clarify the real effect of the treatments are demonstrated . The use of Covariance Analysis to show the pure effect of treatment free from the effect of collateral variables is also indicated. Nested ANOVA that allows division of ANOVA in several parts in order to answer complex questions is described and explained. Finally, the utility of Factorial Analysis for optimization of resource use as well as the illustration of interaction effects are noted and analyzed. (shrink)

Patient competence, defined as the ability to reason, appreciate, understand, and express a choice is rarely discussed in patients with obsessive compulsive disorder, and coercive measures are seldom used. Nevertheless, a psychiatrist of psychologist may doubt whether OCD patients who refuse treatment understand their disease and the consequences of not being treated, which could result in tension between respecting the patient’s autonomy and beneficence. The purpose of this article is to develop a notion of competence that is grounded in clinical (...) practice and corresponds with the experiences of patients with obsessions and/or compulsions. We present a naturalistic case study giving both the patient’s and the therapist’s perspective based on in-depth interviews and a narrative analysis. The case study shows that competence is not merely an assessment by a therapist, but also a co-constructed reality shaped by the experiences and stories of patient and therapist. The patient, a medical student, initially told her story in a restitution narrative, focusing on cognitive rationality. Reconstructing the history of her disease, her story changed into a quest narrative where there was room for emotions, values and moral learning. This fitted well with the therapist’s approach, who used motivational interventions with a view to appealing to the patient’s responsibility to deal with her condition. We conclude that in practice both the patient and therapist used a quest narrative, approaching competence as the potential for practical reasoning to incorporate values and emotions. (shrink)

Proposed educational outcomes for the health humanities in medical education range from empathy to visual thinking skills to social accountability. This lack of widely agreed-upon high-level curricular goals limits humanities educators’ ability to design purposeful curricula toward clear, common ends and threatens justifications for scarce curricular time. We propose a novel approach to the hoped-for outcomes of health humanities training in medical schools, which has the potential to encompass traditional health humanities knowledge, skills, and behaviors while also being concrete and (...) measurable: humanistic practice. Humanistic practice, adapted from the concept of ethical sensitivity, is an intentional process of applying humanities knowledge and skills to a clinical scenario by 1) noticing that the scenario requires humanities knowledge or skills, 2) informing one’s clinical and interpersonal strategy and behavior with humanities knowledge or skills, 3) reflecting on the effectiveness of the strategy and behavior, and 4) reorienting to develop new approaches for future practice. The construct of humanistic practice may help address some of the foundational problems in health humanities outcomes research since it transcends the traditional diverse content domains in the health humanities, can link patient and provider experiences, and may bridge the divide among the additive, curative, and intrinsic epistemic positions of humanities to medical education. In this article the author argues that recognition of self or others is necessary for structural changes in our clients as well as for growth in all relationships. Recognition is seen as a way to deal with our conflicting needs as we progress toward an individuated self and as a way to solve conflicts within our relationships. From a dialectical perspective, dysfunctional interaction between partners reveals both the conflict and the lack of recognition. Various ways of resolving the dilemma are (...) explored. An analysis of a mother-child interaction shows that intersubjectivity is basic in recognition. The dialectic perspective emphasizes in addition the threat and mutuality involved in recognition. With mutual recognition we gain ourselves both as subjects and as objects, and we preserve the relationship. Finally, implications of the dialectical way of conceptualizing relationships are summarily presented. (shrink)

Contrary to the predominant way of doing physics, we claim that the geometrical structure of a general differentiable space-time manifold can be determined from purely dynamical considerations. Anyn-dimensional manifoldV a has associated with it a symplectic structure given by the2n numbersp andx of the2n-dimensional cotangent fiber bundle TVn. Hence, one is led, in a natural way, to the Hamiltonian description of dynamics, constructed in terms of the covariant momentump (a dynamical quantity) and of the contravariant position vectorx (a geometrical quantity). (...) That is, the Hamiltonian description furnishes a natural way of relating dynamics and geometry. Thus, starting from the Hamiltonian state function (for a particle)—taken as the fundamental dynamical entity—we show that general relativistic physics implies a general pseudo-Riemannian geometry, whereas the physics of the special theory of relativity is tied up with Minkowski space-time, and nonrelativistic dynamics is bound up to Newton-Cartan space-time. Christian physicians, nurses and other health care workers must manage a daily conflict of conscience between their Christian faith and predominantly secular health care institutions. This essay examines various efforts for managing these conflicts: a turn towards social justice or a seeking of holiness. Seeking social justice, however, is theologically empty. Traditionally, the Christian requirement that we be “in this world but not of it” requires a journey along a narrow path to holiness. Christian medical morality must, therefore, be understood (...) within this light. However, just as there cannot be generic health care, but rather health care for a particular person's needs and problems, there cannot be generic holiness, but only a holiness grounded in worshiping God rightly. In so worshiping the Christian will be assisted in negotiating the inescapable and perilous vocation of being in the world but not of it. (shrink)

This publication features the stories and experiences of pharmacy professionals who identify as members of historically underrepresented groups. This collection of personal essays presents significant events in the lives of those in the pharmacy community whose experiences have been shaped by their race, ethnicity, gender or gender presentation, sexual orientation, ability, language, mental health, or other factors. The perspectives from the narratives highlight the importance of diversity, equity, and inclusion in the healthcare sector. The authors of the narratives also reflect (...) on their experience to consider what they wish would have happened in the experience, to offer advice for others that may be faced with similar situations. Access to these stories will deepen the collective understanding among pharmacy colleagues, patients, and society of the experiences that individuals from historically underrepresented groups within pharmacy have experienced. Features of threatening cues and the associated context influence the perceived imminence of threat and the defensive responses evoked. To provide additional knowledge about how the directionality of a threat (i.e. directed-towards or away from the viewer) might impact defensive responses in humans, participants were shown pictures of a man carrying a gun (threat) or nonlethal object (neutral) directed-away from or towards the participant. Cardiac and electrodermal responses were collected. Compared to neutral images, threatening images depicting a gun directed-towards the (...) participant induced sustained bradycardia and an increased electrodermal response, interpreted as immobility under attack. This defensive immobility reaction is evoked by high perceived threat and inescapable situations and indicates intense action preparation. Pictures of guns directed-away from the participant induced shorter bradycardia and no significant modulation of the electrodermal response compared to neutral pictures, which might be consistent with the perception of a less threatening situation. The results show that the directionality of threat stimuli is a key factor that prompts different patterns of defensive responses. (shrink)

Recent work in philosophy could benefit from paying greater attention to empirical results from cognitive science involving judgments about the nature of our ordinary experience. This paper describes the way that experimental and theoretical results about the nature of ordinary judgments could—and should—inform certain sorts of enquiries in contemporary philosophy, using metaphysics as an exemplar, and hence defines a new way for experimental philosophy and cognitive science to contribute to traditional philosophical debates.
